Consent and Waiver: 2019 Guatemala Mission
This consent form gives permission to seek whatever medical attention is deemed necessary, and releases Medical Mission of Mercy USA (MMOM USA), its Directors, and officers, and trip leaders of any liability against personal losses.
1. I understand that there are inherent risks involved in any mission trip and I hereby release MMOM USA, its Directors, officers, and trip leaders from any and all liability for any injury, loss, or damage to person or property that may occur during the course of my involvement in the mission. In the event that I am injured and require the attention of a doctor, I consent to any reasonable medical treatment as deemed necessary by a licensed physician. In the event treatment is required from a physician and hospital personnel designated by the Mission Chief Medical Officer, I agree to hold such person free and harmless of any claims, demands of suits for damages arising from the giving of such consent. I also acknowledge that I will be ultimately be responsible for the cost of any medical care should the cost of that medical care not be reimbursed by my health insurance provider(s).
2. I affirm that I have informed the Mission Chief Medical Officer of any pre-existing conditions or other medical information that MMOM USA should reasonably be aware of.
3. I agree to bring myself home at my own expense should I become ill and/or if it is deemed necessary by the Mission Leader at his/her discretion
4. I have consulted with appropriate medical professionals and have obtained the appropriate inoculations for this trip
5. I acknowledge that photography and/or videography may take place during any event or activity and, by consenting to this waiver, grant express permission to the MMOM USA for the use of the my likeness as it sees fit to support its mission and without compensation.

1. Volunteers are responsible for arranging their own funding for the costs of the trip to including but not limited to travel expenses, lodging (prior to and after the mission dates) , insurance of any kind including trip cancellation, medical and/or evacuation, and immunizations.
2. The major costs of the trip, with the exception of each volunteer's air transportation from their home to Guatemala City Aurora airport (GUA), will be managed by MMOM USA This includes lodging for the full duration of the mission trip, food during the Mission unless indicated otherwise, and air and ground transportation/transfers between GUA airport, the hotel, and the mission site. The costs of voluntary participation may be tax deductible. The Medical Mission of Mercy USA is a non-profit 501 (c) 3 organization in the State of Maryland. Volunteers should consult their tax advisor on this matter.
3. Volunteers are responsible for coordinating their flight itinerary. Each volunteer will be informed in advance of the pre-determined meeting place. Upon arrival at the predetermined location, MMOM, USA will make the necessary arrangements to and from the mission site. If a volunteer is not able to meet the group at the pre-determined location and / or cannot stay for the full duration of the mission, they will be responsible for their own arrangements to and from the mission site. Volunteers are encouraged to discuss all alternative arrangements with MMOM, USA. Deadlines are established and must be adhered to at all times including but not limited to paperwork and payments. In the event a volunteer cancels after paying, MMOM USA will make reasonable efforts to secure refunds from their vendors but payments may be non-refundable in full or in part at the sole discretion of MMOM USA.
